To fix this problem: 1. Do not use cable to connect to the PS5 console. 2. If you are using a cable to connect to the PS5 console, you need to re-plug the USB adapter and re-pair it with the controller (must be re-paired).

Any other problems you encounter, set up the host computer as in the answer to question 1, then re-pair the USB adapter with the controller, this solves 99% of the problems.
